Forms

All forms must be completed and turned in by Monday, August 3rd (preferably earlier, if possible). If athletes do not have these forms turned in, they should still attend practice so we can take their attendance, give them an alternate workout, and then have them run on their own (until everything is turned in). Athletes are still a part of the team, but cannot participate in team activities until these forms are completed and turned in.

All forms must be completed and uploaded to FINAL FORMS. An email was sent out by the district regarding this.

The two most important forms are the MEDICAL FORM (which goes to the coaches) and the PHYSICAL FORM (which goes to the Learwood Office). if you need hard copies of these, here are the links to both:
