Forms

All MEDICAL FORMS & PHYSICAL FORMS must be completed and turned in by Friday, August 1st (preferably earlier, if possible). If athletes do not have these forms turned in, they should still attend practice so we can take their attendance, give them an alternate workout, and then have them run on their own (until everything is turned in). Athletes are still a part of the team, but cannot participate in team activities until these forms are completed and turned in.

The two most important forms are the MEDICAL FORM (which goes to the coaches) and the PHYSICAL FORM (which goes to the Learwood Office). Here are the links to both:

OTHER FORMS:

• Currently, the ALHS page that has links to all necessary forms doesn't seem to work, so don't worry about the other forms
• The Medical Form & Physical Form are the ones we really need for athlete participation
• Don't worry about Pay-to-Participate Form yet...we'll get clarification on that once we get closer to school, and typically that's not due until mid-September
