Forms

All MEDICAL FORMS & PHYSICAL FORMS must be completed and turned in by Monday, August 1st (preferably earlier, if possible). If athletes do not have these forms turned in, they should still attend practice so we can take their attendance, give them an alternate workout, and then have them run on their own (until everything is turned in). Athletes are still a part of the team, but cannot participate in team activities until these forms are completed and turned in.

The two most important forms are the MEDICAL FORM (which goes to the coaches) and the PHYSICAL FORM (which goes to the Learwood Office). Here are the links to both:

The Athletic Department website has everything accessible on one page. We've highlighted the forms we need below. We really just need the Physical Form & Medical Form filled out for athletes to practice (other forms can be filled out and turned in, but athletes can still practice while this is getting done). We've broken down pertinent forms and the sections they're in here:

READ SECTION:

• Click on each link and read each corresponding document
• Pay-to-Participate form must be accessed, printed out, filled out, and returned to the Learwood office. NOTE: Parents, you have time to complete this form. The deadline is not yet established, but it will most likely be some time in the first week of September.
• Physical forms must be accessed, printed out, completed by a physician, and returned to the coaching staff. We recommend dropping off this form to the Learwood office and the Athletic Director will process it.

SIGN & RETURN SECTION:

• Digital signatures are required for the Parent-Athlete Agreement Form & Concussion Sign-Off Form.
• Emergency Medical form must be accessed, printed out, filled out, and returned to the coaching staff. Athletes cannot participate on August 2nd without this form completed and turned in.
• Pay-to-Participate form is a repeat of the one listed above. No need to access it again.

STATEMENT of ACKNOWLEDGMENT SECTION:

• This section is where you digitally sign the forms in the preceding section.
